2) Price per linear foot on 5" baseboards?
3) Price per linear foot on simple/standard crown molding?

This brings to mind an old favorite image...

I'd strongly suggest considering doing it yourself.  It's incredibly easy with just a couple of needed tools, and very inexpensive materials.
